According to Ahlbäck system, knee joint osteoarthritis is classified as: • grade 1: joint space narrowing (less than 3 mm) • grade 2: joint space obliteration • grade 3: minor bone attrition (0-5 mm) • grade 4: moderate bone attrition (5-10 mm) • grade 5: severe bone attrition (more than 10 mm) 2012-10-09 · Controversy exists regarding the relationship between radiographic findings and clinical status in knee osteoarthritis. Although the surgical indication for total knee arthroplasty (TKA) should be based on pain, clinical status, and the deterioration of quality of life, the radiographic study is the most commonly used criterion for preoperative evaluation. Thirty-seven knees had grade-1 Ahlback osteoarthritis in the lateral compartment on the preoperative radiographs, and one had grade-2 Ahlback osteoarthritis. The preoperative skyline view of the patellofemoral joint showed no bone loss with eburnation and longitudinal grooving in all the cases. In severe osteoarthritis patients (Ahlback grade 3, 4, 5), this treatment was of less benefit if those patients were young, active, and expected independent ambulation. Surgical treatment may be a procedure of choice to meet patient expectation in improving function and ambulatory status.
We divided patients with Ahlback grade 1 or 2 osteoarthritis into the three groups of single and double injection of PRP and three injections of hyaluronic acid.

Therefore, the trials by Moseley et al and Kirkely et al provide strong evidence that patients with symptoms attributable to knee OA per se, and not meniscal tear, do not improve following arthroscopic lavage In general, higher grades of osteoarthritis were associated with more severe articular cartilage degeneration in all three radiographic grading scales. For the Ahlback and Brandt grading scales, the mean numeric values representing the severity of articular cartilage degeneration increased with each increasing radiographic grade of osteoarthritis. Correlation coefficients describing the relation between grade of osteoarthritis and severity of articular cartilage degeneration were calculated for each grading scale.
